The dental practice retains the right to interpret any program stipulations.

The annual membership fee must be paid in full prior to the treatment.

Each additional family membership must be paid at the time of the initial membership or at the renewal time.

No refunds will be given in the event Member terminates the plan prior to the end of the plan year.

Membership benefits are not transferrable, have no cash value and may not be redeemed for cash.

This is not an insurance plan and is not subject to regulation by the state department of insurance. Our membership plan is not (i) insurance, (ii) a qualified health plan under the Affordable Care Act, (iii) pre-payment for future services, or (iv) payment for access to discounted services. Rather, our membership plan is a direct payment arrangement for services, products and care provided.

Our membership plan does not cover procedures completed by specialists outside or inside our practice.

Plan membership can only be used at RJ Dental Smiles location only.

Plan membership cannot be combined with current dental insurance plans.

Membership fee may be adjusted annually.

Missed appointment fees/penalties are ineligible for the membership discount. However, if a scheduled free cleaning and exam is scheduled and that appointment is missed without notification to the office that service will be null and void.

Total payment amount is due at the time of services are provided. If full payment is not received at the time of service, fee discounts will be void.

10% of Invisalign services, cannot be combined with any other promotions.
